* All parishioners registered at Mary, Mother of Mercy Parish who are seeking faith formation for their children must fill out the "Lifelong Faith Formation 2025 /2026" form below.
* Those preparing to receive a sacrament in the upcoming calendar year ALSO need to fill out the "Sacramental Preparation 2025 / 2026" form.
Please note: At least two years of faith formation are recommended leading up to reception of a sacrament. Typically, this will be 1st & 2nd grade to receive 1st Holy Communion, and 7th & 8th grade to receive Confirmation. Delayed enrollment in faith formation may delay the reception of a sacrament.
